We’re excited to announce our April Illuminating Impact winner, Harbor Care!

Harbor Care is a New Hampshire nonprofit working to break the cycle of homelessness through housing, healthcare, and veteran services. Their mission is rooted in a simple but powerful belief: everyone deserves access to safety, stability, health, purpose, respect, and dignity. Through integrated programs across the state, Harbor Care helps individuals and families facing life’s toughest challenges find the support they need to move forward.

How Harbor Care Helps Build Safe, Stable, and Healthy Lives

Harbor Care provides vital services to people across New Hampshire, including individuals and families experiencing homelessness, people living with disabilities, those facing substance use challenges, and veterans in need of specialized support.

Their housing programs help people access long-term stability through permanent supportive housing, transitional housing for veterans, recovery housing, and wraparound support services. These services go beyond housing alone, offering case management, mentoring, life skills training, health and well-being support, eviction prevention, and other tools that help people build lasting independence.

Harbor Care also provides comprehensive healthcare designed to meet people where they are. Their services include primary care, preventive care, dental care, mental health care, substance use treatment, HIV/AIDS services, pharmacy support, and community health programs. With interpreters, telehealth options, Nashua-based care locations, and a sliding fee scale, Harbor Care works to make healthcare more accessible regardless of income or insurance status.

For veterans and their families, Harbor Care offers specialized programs focused on housing, employment assistance, mental health support, benefit coordination, suicide prevention, and homelessness prevention. Their veteran services include transitional and permanent supportive housing, along with programs that help at-risk veterans access financial aid, case management, and critical resources.

In 2025, Harbor Care helped more than 5,000 individuals and families access housing, healthcare, veteran services, and other vital care. That impact includes more than 1,000 individuals and families accessing or maintaining stable housing, more than 2,300 individuals receiving healthcare services, and more than 640 veterans and family members accessing stable housing, mental health care, and suicide prevention support.
